Participants will learn to differentiate hospice from palliative care and understand the distinct benefits of each, when each service is appropriate in an individual’s health trajectory and how to access both using case studies.  Additionally, the common health care trajectories will be reviewed highlighting how decline can look different depending on the health conditions.  Finally, we will review advance care planning, why this is helpful for patients, their families, and their health care providers to ensure goal concordant care.
